Help for Ordering By Fax Or Phone

Click on Fax Order Form and when the form displays, use your browser's print function.
Find your items (Search, is a good place to start). The Store Front page and main product list are also helpful. Enter product and price info on the printed Fax Order Form.
If you want to phone your order in toll-free, call (888) 856-9970, please be ready to provide the information requested on the Fax Order Form,
To fax in your order, simply complete the printed form and fax it to our secure fax number: 415-621-4811.
If payment is made by check or money order, a completed form must accompany the order. Such orders should be sent to the address indicated at the bottom of the Fax Order Form.

Notes on completing the Fax Order Form:

	BOLD fields are Required.
	If the Billing Address is the same as the Shipping Address, you do not need to complete the Shipping Address section.
	Please supply your card number AND the additional security code (found on your card as shown, below):

Visa, Master, and Discover Cards • 3 digits in length • On back of card, immediately following the account number in the signature panel	American Express • 4 digits in length • On face of card, above and to the right of the card number
American Express Optima • 4 digits in length • On face of card, above and to the left of the card number	American Express Blue • 4 digits in length • On face of card, above and to the right of the card number

Please complete the expiration date of the credit card using Month Number and Year Number.

Use the product codes and prices from our latest price list (or from our online store). In most cases, online store will be used as
the official guide when it comes to pricing.
