A Better Health, Nutrition, and Vitamin Store  (egetbetter.com) is a distributor of the Personal Health Response, Inc. (PHR)  KAMM-12 report and we are working with PHR to facilitate report processing for individuals interested in having the KAMM-12 report done.

Here's how you can order the KAMM-12 (you might want to print this page and use it for future reference).

1. Go to the Online Store (click the yellow bar at the top of this page).

• Since you're probably new to the egetbetter.com web site, you will need to click on "Account" in the store front navigation bar, then click on "Create New Account" and enter in the required information, then click on the save button at the bottom right hand side of the form. 

• After setting up your account, click on "Metabolic Reports" to order the KAMM-12 (Product Code PHR-K12-001).

2. Once ordered, you will receive your KAMM-12 Test Requisition to take with you to a nearby diagnostic laboratory for your blood to be drawn and tested and your KAMM-12 generated.

• Laboratory results are sent to Personal Health Response, Inc. for analysis and generation of the KAMM-12 report.

• Your report will be shipped to you based on the method you selected for shipping

3. In your report will be an individualized list of suggested nutritional and behavioral improvement steps.  We will include with your report  a list of our products which match the report's recommendations.  You can fax-in the order, phone-in the order or even use our online store to order the products..
